ADMISSIONS
Surgical
Your admission to Calvary John James Hospital will be co-ordinated
by your surgeon, who will provide you with the required admission
forms. If you have any questions regarding your operation you should
discuss these with your surgeon at the time of completing the forms.
These forms need to be returned to the Admissions area at the hospital
to confirm your booking.
A notification letter will be sent to you confirming your admission
and providing you with an estimate of costs for your procedure.

Medical
Your physician co-ordinates your medical admission and will notify
you of your admission date and time.
Please present to the hospital Reception when you arrive.
Maternity
Your doctor will refer you to an obstetrician who has visiting
rights to the hospital. The obstetrician will give you forms to
complete. These forms need to be returned to the
Maternity Centre
as soon as possible. The hospital's admissions staff will process
your paperwork and send you a confirmation letter outlining your
out-of-pocket expenses, visiting hours, and education classes.
When you go into labour, or if you have any concerns, you should
ring the Delivery Suite on telephone (02)6281 8732. The Delivery
Suite staff will advise you when you should come to the hospital.
What to Bring
- Personal toilet articles
- Sleepwear, gown and slippers
- Sufficient cash for incidental items
- All current medications in their original containers
- All current x-rays and scans
- Health insurance membership details
- Medicare card and any other concession cards
- Full details of Workers’ Compensation and Third Party Insurance,
if applicable
- Patient Pathway, if already provided by the hospital
Medications
Please bring all your current medications with you. Your doctor
will order all medications you need whilst in hospital. Additional
medications required will be dispensed by the hospital pharmacy.
Continue to take your
normal medications up to the time of your admission, with a small
sip of water, unless otherwise advised by your doctor.
Any pension or pharmaceutical entitlement number should be lodged
prior to or at admission.
On discharge all medications should be collected from the nursing
staff.
What Not to Bring

Do not bring articles of value as the hospital does not accept
liability for loss of personal articles.

For your own safety all jewellery should be left at home, prior
to coming to hospital. If necessary, your wedding ring may be
taped in place. All medic alert bracelets, etc should be left
on.

You will be asked to remove any contact lenses / glasses and
all cosmetics, including nail polish and eye make-up. If you
have a hearing aide, this can be left in.
Children Undergoing Day Surgery or Staying
Overnight
Parents play an important role in preparing children for what to
expect before and after surgery. If children are well prepared,
they will have a better surgery experience.
A pre-admission tour of the Ward is available and can help you
and your child prepare for surgery. Please phone the hospital on
(02) 6281 8100 between 9.00am and 4.00pm, Monday to Friday, to arrange
a suitable time.
