New Appointments
Doctors or Allied Health Professionals who wish to obtain clinical
privileges at Calvary John James Hospital are required to complete
an
application form.
The outcome of the Council's decision will be notified to the applicant
in writing.

Re-Appointments
Calvary John James Hospital
requires, under the LCMHC By-Law, that every three years
(or one year for VMO’s over the aged of 65), all visiting practitioners
apply in writing to the Medical Quality Council for re-appointment.
This should be done on the required
Re-Application
Form.
